Privacy Policy
We require our employees to sign a confidentiality statement which will result in termination if breached in order to ensure that your privacy is protected. We do not conduct medical business by email and we ask that you do not correspond with the doctor via email regarding health related matters. Information given to or recorded by the doctor will go in your medical record. The physical record belongs to the physician, but the information contained in the record belongs to you. At your first visit, we will ask you to consent to release of the information for treatment necessary for your condition, payment from a third party carrier, and to carry out normal healthcare operations. Detailed examples are found on the back of the patient registration, a copy of which will be given to you. If you choose not to consent to the release of information for treatment, payment, or healthcare operations, Dr. Strinden will not be able to see you and the professional relationship may be terminated.
A separate written authorization will be necessary to release your medical records either to you or a third party. We may require 14 days to locate and copy your records depending on their accessibility. We may require a copying charge for excessive copies. You have the right to request an amendment or correction to your protected health information, but only if the request is made in writing.
